Volunteer agreement and release form

 

 

By signing and submitting this application, I am asking to be considered as a volunteer of SBPA Services Inc. I the event that I am accepted as and become a volunteer, I agree to abide by the rules. Policies and procedures of SBPA Services Inc. and be a representative of its mission. When I accept volunteer work, I will abide by the schedule and perform the tasks assigned. When volunteering I will act in a humane manner toward all animals and people with whom I interact.

 

I fully understand and agree that my services are provided strictly in a VOLUNTEER capacity. I fully understand and agree to assume all risks involved in any and all duties that I perform for SBPA Services Inc. in my volunteer capacity. Such duties may include, but are not limited to: animal handling, shelter/cage assistance, custodial work and other foreseeable volunteer duties. I fully understand that SBPA Services Inc. expects high standards of moral and ethical treatment of all animals including, but not limited to, the animals under its care. I agree to adhere strictly to these standards in my volunteer capacity. I fully understand agree to comply with any and all of the obligations outlined in this Volunteer Agreement. For any reason whatsoever, while performing my volunteer services, SBPA Services Inc. may terminate my service without cause. On behalf of myself, my heirs, personal representatives, executors and administrators or anyone else who might claim on my behalf, I hereby release, discharge, indemnify and hold harmless SBPA Services Inc., it’s agents, servants, volunteers and Board of Directors from any and all claims, causes of actions or demands of any nature or cause connected with my Volunteer Agreement and service. I allow SBPA Services Inc. to use any photograph taken of me for use in public relations efforts and hold SBPA Services Inc. harmless for any and all lose or damage to my personal property while performing volunteer services for SBPA Services Inc.

I acknowledge that I have read and fully understand the terms and conditions of the foregoing Volunteer Agreement and that I will comply with the same.
